• All clients must complete a consultation form prior to receiving TLLTM treatments. • We collect only relevant information that is necessary for the safe administration of treatments • No personal information from consultation forms is stored on any computer • Client information will not be shared with third parties or anyone other than the client unless the client is under the age of 18 or has a care worker or guardian • Consultation Forms will be stored securely by TLLTM for one year and then securely destroyed by shredding • Emails are retained for communication purposes for 1 year and then deleted but Offensive or inappropriate communications are retained for the protection of our staff and blocking purposes. • Right to be forgotten – You may request destruction of your consent form at any time via email or writing to us • You may request a copy of your data via email or in writing to us, once we have verified your identity we email it to you
